Affidavit Update Packet for Non-U.S. Grantees
This packet, required for all non-U.S. grantees, updates your organization's original affidavit to the Foundation. This packet is required to be completed approximately every two years. The packet contains forms necessary for completing an affidavit update, including instructions, the public support schedule, and major donor support form. (Please contact your Mott Foundation program officer if you have any questions regarding the completion or due date of this packet.)
Project Support Financial Report
In accordance with the grant agreement, grantees should use this form to report expenditures, funding sources and other financial matters for project support grants. The financial report must be submitted with the narrative report.
Project Support Narrative Report
In accordance with the grant agreement, grantees should use this form to report on progress toward the stated goals and objectives of a project support grant. The narrative report must be submitted together with the financial report.
Redistribution Statement for General Purpose Grants
Certain grantees (other private foundations or private foundation equivalents) should use this form to certify how general purposes funding from the Mott Foundation was spent. The commitment letter will inform the grantee if and when this form is required.
Redistribution Statement for Project Support Grants
Certain grantees (other private foundations or private foundation equivalents) should use this form to certify how project support funding from the Mott Foundation was spent. The commitment letter will inform the grantee if and when the form is required.
Request for Wire Transfer of Funds
This form contains all the required information for the Foundation to make a wire payment. Please note that this payment option is available only to overseas grantees.
